There is an Accident and Incident Checklist for you to follow if an incident has occurred.
 
Accidents happen.  But the same accident shouldn't happen twice.  That's why it's vital that accidents (or incidents which could have resulted in an accident) are reported, investigated and any lessons learned.
 
Who needs to read this?
This guidance is for everyone who works for the BBC. It tells you why accidents, incidents and cases of ill-health need to be reported and investigated and how to do it. It describes what the BBC means by different terms such as Accident, Incident and near miss and it explains the difference between reporting and investigation.

What needs to be reported?
Accidents and incidents should be reported wherever they happened: in a BBC building; on location; on overseas assignment or anywhere else. But only if they are directly connected with the BBC's work. Every report of and accident or incident must be reviewed, investigated and signed off by a manager.
