Who issues a 'hot work permit'?
A responsible person with sufficient competence to oversee the operation and cause checks to be made of the area after the hot work has been completed.
 
What happens after the hot work has been completed? 
The area is inspected by a competent person to check for fire and the permit is signed off by the issuing person.
